UIAlertView has some different styles that can be applied to it. The UIAlertViewStyleLoginAndPasswordInput creates a simple login popup.

alert.AddButton ("Cancel");
alert.Message = "Please enter your login details";
alert.AlertViewStyle = UIAlertViewStyle.LoginAndPasswordInput;
alert.Clicked += ButtonClicked;
alert.Show ();
